What clinical notes look like in SideClick

A note in SideClick is a structured SOAP document attached to a session: subjective, objective, assessment and plan, each its own field rather than one undifferentiated text box. The note sits on the patient's timeline next to the appointment it documents, the invoice it produced and the outcome measures it references, so reading back a case is scrolling one record, not reconciling four tools.

Drafts are kept safely until you sign. If the day gets away from you mid-note, nothing is lost; the draft is waiting on the record when you come back to it that evening or the next morning.

Signing, amendments and the audit trail

Signing a note locks it. From that moment the original text is preserved: corrections happen as amendments layered on top, each one attributed and dated, so the record always shows what was known and documented at the time. There is no silent editing of history, which is exactly the property regulators, lawyers and insurers ask about first.

Underneath the notes sits a tamper-evident audit trail. Every read and every write is logged, which turns "who accessed this record and when" from an uncomfortable investigation into a query the software answers in seconds.

How it works in practice

A typical session: open the appointment from the day sheet, and the chart is already in front of you with the last note and the care plan goals visible. Document during the session or dictate afterwards; if you have opted in to on-device AI, the assistant drafts the SOAP sections from the session's own context and runs a quality check that flags gaps before signing. You edit, you sign, the note locks, and the audit trail records the lot.

Where your notes live, and how they leave

Notes are stored in an encrypted database on the practice's own hardware, not in a vendor cloud. When a note needs to travel — to a referrer, a funder, a lawyer or a patient — it exports as PDF, FHIR or CSV from the chart.
