Measurement built into treatment

Outcome measures in SideClick are captured where care happens: scores recorded in session land on the chart next to the note that explains them. PROM forms go to patients through the in-office kiosk in the waiting room, or at home through the patient portal, so the data arrives before the review instead of eating its first ten minutes.

Because measures live on the same record as everything else, the question "is this working?" has a chart to point at rather than a feeling to defend.

Care plans with goals that move

An episode of care is structured around goals and the plan to reach them. Progress is tracked against those goals across sessions, and if you opt in, the on-device assistant reads your session notes for evidence of goal progress and lines it up against the plan. It proposes; you confirm what actually moved. No status ever changes without a clinician's say-so.

How it works in practice

Intake sets the baseline measures and the episode's goals. Each session adds its scores in seconds, the patient completes a PROM at home the night before each review, and the review itself opens on a trend chart of the whole episode. When the funder asks for evidence at week twelve, the report drafts itself from data that was captured as a side effect of treating.

From trend to report

Progress charts across the care plan show whether the intervention is working while there is still time to change it. When the episode needs to become a document — for a referrer, a school, a funder — the opt-in report assistant produces a first draft from the record, and it exports as a PDF you shaped and signed.

Which outcome measures does SideClick support?

The measures your discipline already uses: SideClick records scores and structures them over time rather than licensing a fixed instrument library. If you can score it, you can track it.

How do patients complete PROMs at home?

Through the patient portal on Portal editions: the practice sends the form, the patient completes it on their own device, and scores land on the chart before the session. In the waiting room, the kiosk covers the same job in every edition.

Can the AI change a goal's status?

No. If you opt in, it extracts progress evidence from your notes and suggests updates; a clinician confirms every change. The care plan is never edited autonomously.

Can I export outcome data for research or audits?

Yes — CSV for analysis, PDF reports for humans, FHIR for other systems, plus a de-identified export with automatic redaction for sharing beyond the care relationship.
