Scoring

Each item scores 0 to 3; the total is the sum. Bands follow the validation study: 0 to 4 minimal, 5 to 9 mild, 10 to 14 moderate, 15 to 21 severe. A total of 10 or more is the usual threshold for further assessment.

FAQ

Questions, answered

Can I give this to a patient to fill in?

The page is built for the clinician scoring a completed form. Inside SideClick the same measures go to the patient portal to complete before the appointment and score onto the record automatically.

Is a band a diagnosis?

No. It is a severity band from a screening measure, to be read with the person's history and your clinical judgement.
