Kroenke K, Spitzer RL, Williams JB. The PHQ-9: validity of a brief depression severity measure. J Gen Intern Med. 2001;16(9):606–613. Developed by Drs Spitzer, Williams and Kroenke with an educational grant from Pfizer; no permission is required to reproduce, translate, display or distribute.

Scoring

Each item scores 0 (not at all) to 3 (nearly every day); the total is the sum. The band follows the validation study: 0 to 4 minimal, 5 to 9 mild, 10 to 14 moderate, 15 to 19 moderately severe, 20 to 27 severe. The page scores only; it does not decide anything.

Item 9

Any answer above "not at all" on the self-harm item is surfaced above the total, whatever the score. Follow your service's risk process. The page does not store the answer.

FAQ

Questions, answered

Can I give this to a patient to fill in?

The page is built for the clinician scoring a completed form. Inside SideClick the same measures go to the patient portal to complete before the appointment and score onto the record automatically.

What about the tenth question on difficulty?

The functional-difficulty question is not part of the score and is not on the page; ask it in the room.
