What each sheet is for

The intake sheet collects who the person is, why they have come in their own words, the history that shapes the plan, and the practicalities — languages at home, who attends, best times. The SOAP sheet gives one session four sections with room for the data in the objective block and the home practice under the plan. The session-planning sheet puts each goal in a row with its target and criterion, the activity and materials, and how the data will be taken, so the note writes itself afterwards.

They are deliberately plain: no logo block, a practice line to fill in, and ruled lines sized for handwriting.

Why there is no consent form

Consent wording carries legal weight and differs by regulator and country. A generic template would be wrong somewhere, and wrong in a way that matters. The intake sheet points to the practice's own consent form instead.

From paper to the record

A completed sheet is a document to attach to the chart, or the source for a typed note. Inside SideClick the same structures are the patient record, the note editor with its pre-sign checks, and the care plan.
