Session data tally
Big tap targets for independent, cued and incorrect responses across as many targets as the session needs, a timer, an undo for the mis-tap, and a one-line summary per target you can paste straight into the note.

The tally is kept in this browser only, so a locked screen or refresh loses nothing. Clear session removes it.

Session data Target 1: no trials
Built for one hand and a child in the room
Each target gets three large buttons that work with a thumb on a phone: independent (counted as correct), cued, and incorrect. The running line under them shows correct out of trials with the percentage, and the percentage with cues when any were given. Undo takes back the last tap on that target only.
Name the targets whatever you track — a phoneme and position, a language target, a step in a task analysis, a balance exercise. The summary uses the names as typed.
The summary that goes in the note
The copy button produces one line per target in the form "14/20 independent (70%), 3 cued, 3 incorrect, 85% with cues", plus a session total and the duration when the timer ran. Paste it into the objective section of a SOAP note or into whatever data sheet the practice keeps.
What it deliberately does not do
It does not know who the patient is, and does not ask. There is no login and nothing leaves the device. Keep the tally for one session and clear it when the note is written.
- No patient name or identifier — the summary is text you attach to the right record yourself.
- No history across sessions; the app does that against the care plan.
Questions, answered
Can I add it to my phone's home screen?
Yes. In Safari or Chrome use Share, then Add to Home Screen; it opens like an app and keeps the last session until you clear it.
What counts as a trial?
Every tap on one of the three buttons is one trial. Independent responses count as correct; cued responses are shown separately and included in the "with cues" percentage.
