In SideClick: Care plans
Care Plans is on a computer only. Choose the patient, then the plan; each plan is named by its start date and status. New plan starts a plan for the patient, active from today. Complete closes an active plan, and the bin deletes it.
The plan's card shows when it started, its target end if one is set, and how many of its goals are at target.
Goals & progress lists the plan's goals. Add goal names a goal with a description, the unit it is measured in and a target value; Edit changes it or marks it Achieved or Abandoned. Log records a measurement for a goal, the value and an optional note, and the goal's line then shows the latest value, the change since the first, and the target, with a small chart beside it. Target met appears once a measurement reaches the target.
Outcome measurements (PROMs) shows each instrument the patient has answered, with the number of responses, the latest score and a chart of the series.
Referrals opens the referrals screen, where a referral is written against a care plan.
A session note can also suggest measurements for a goal with the AI; each is saved only when you confirm it there.
