A patient's record

Everything about one patient on one page: their details, sessions, who can see the record, consent, documents and reports.

In SideClick: Patients → a patient

The record opens with the patient's name and file number. A copy from another practice says so under the name, your own copy from a practice you left or a copy sent to you, and its earlier entries are read-only history. Caseload insights for this patient show as chips below the name.

  • Demographics shows what intake recorded. On a computer, Contacts and Insurance follow: Add policy records a policy, Make primary chooses the main one.
  • Recent sessions lists the latest. New session starts one; on a computer it asks when the session took place, so a note typed up later carries that date.
  • A notice appears when the record is past a retention period you are responsible for; Keep or Delete is chosen under Retention.

Who can see this record lists every cover, handover and assignment on the file, with End where you may end one, and the copies sent from it. The treating clinician or a practice owner can Send a copy to a clinician of another profession, Hand over to a clinician outside the practice, Change treating clinician to a colleague of the practice, Assign the patient to an assistant, or Discharge patient. Sending, handing over, changing and discharging are done on a computer, where the patient's consent is recorded. A cover is arranged under Settings → Practice and people.

On a computer, AI note generation records or withdraws the patient's consent to AI help, Progress & treatment reports and Ask this client use the on-device AI, and Documents keeps files on the record. In the Practice edition, Patient portal invites the patient to the portal and Home program publishes a plain-language summary to it.

  • New patientOpen a patient file: the patient's identity and contact details, then the purposes they consent to.
  • SessionWhere a session is timed and its note is written: raw notes or dictation on the left, the structured draft on the right, then Review & Sign.
  • Consent ManagementWhat each patient has agreed to, purpose by purpose: grant, withdraw and read the history.
  • Who sees whatA patient file has three layers — identity, money and clinical — and your hat in the practice decides which reach you; a grant adds patients, never layers.
  • Cover while you're awayA colleague of your profession in your practice sees and writes in your patients' records from a date until a date; when the cover ends, what they borrowed is destroyed and their own notes stay.
  • Sending a copy to another professionA copy is sent once, on the patient's recorded consent: the other clinician holds the patient's details and the signed entries you chose, read-only, and nothing written afterwards reaches them.
  • Handing over a patientA patient moves to a clinician at another practice with their history: the earlier entries are read-only for the receiver, who adds their own after them.
  • Changing the treating clinicianInside the practice a patient passes to a colleague by naming them the treating clinician: they receive every key of the file and continue it, with the earlier entries underneath.
  • Assistants and co-signatureAn assistant works under a named supervising clinician, sees only the patients assigned to them, and writes notes that wait for the supervising clinician's co-signature or come back for changes.
  • Keepers and retentionEvery copy of a record has one keeper, who types their own number of years; when a record passes it, the keeper chooses Keep or Delete, and the app never deletes on its own.
  • The patient portal and the front-desk kioskThe kiosk is a computer at reception where a patient fills in forms; the remote portal is where patients message, book and read from home, in the Practice edition.
  • The AI featuresDrafts, summaries, insights and dictation run on the computer itself — decision support, never a diagnosis — and only with the patient's consent.
