A note

Read a note, sign or amend it, co-sign an assistant's note or return it, attach files and export a signed PDF.

In SideClick: Notes → a note

The header shows the note's framework, its status, when it was started and who wrote it. A signed note shows Verified when its signature checks out, or Tampered! when the content no longer matches the signature. An imported note is a read-only historical record.

  • Edit Draft reopens your own draft in the session.
  • Sign Note opens Review & Sign: the note, a few things to check, which are suggestions only, and your confirmation. Only the author signs their note; a draft by someone who has left the practice is frozen, and nobody else can sign it.
  • Amend Note, on a signed note, makes a new draft with a copy of the content and an optional reason. The original stays read-only, marked Amended, and both appear under Version history.
  • Export PDF, on a computer, writes a signed PDF. The file is the full record, not a de-identified copy.

An assistant's note is sent for co-signature and waits in Review. The supervising clinician sees Co-sign, which signs it with the same confirmation, and Return for changes, which sends it back as a draft with a reason the author reads on the note.

Attachments are files on the note, sealed under the patient's key. On a computer, Attach file picks a file; on a phone or in the browser you capture a document or a voice memo instead, and a voice memo can be transcribed later on a computer where dictation is available. When two devices edited the same section, the conflict is shown above the note until you choose a version or merge them.

A note handed over to you is read-only: add your own entries after it.
